SUPPORTED LIVING - SUNNYHILL OFFERS SUPPORTED LIVING 365 DAYS A YEAR FOR CHILDREN AND ADULTS WITH DEVELOPMENTAL DISABILITIES SO THAT THEY MAY LIVE WITHIN THE COMMUNITY OF THEIR CHOICE. STAFF PROVIDES CONSULTATION, TRAINING, AND SUPPORT AS NEEDED.
SUNNYHILL INDEPENDENCE CENTER ASSISTED LIVING FACILITY (ALF) - THE ALF HAS FIVE RESIDENTIAL BUILDINGS WHICH CAN HOUSE UP TO 32 INDIVIDUALS WHO ARE UNABLE TO LIVE BY THEMSELVES BUT DO NOT NEED SKILLED NURSING CARE. THE ALF PROVIDES SERVICES SUCH AS ROOM AND BOARD, MEDICATION ADMINISTRATION AND PERSONAL CARE ASSISTANCE.
INDEPENDENT SUPPORTED LIVING ASSISTANCE (ISLA) - THE ISLA PROGRAM PROVIDES ONE-ON-ONE SERVICES TO INDIVIDUALS IN THEIR OWN HOMES. SERVICES PROMOTE SKILLS FOR INDEPENDENCE, FORMATION OF SOCIAL ROLES, RELATIONSHIPS, AND SELF-RELIANCE.THIS SERVICE CAN INCLUDE UP TO 40 HOURS OF SUPPORT A MONTH. SUNNYHILL CURRENTLY SUPPORTS OVER 273 PEOPLE THROUGH THIS PROGRAM.QUALIFICATIONS INCLUDE: DD DIAGNOSIS PRIOR TO AGE 22, CAPACITY TO LIVE INDEPENDENTLY AND RESIDE IN FUNDING COUNTY.
