THE PRIMARY EXEMPT PURPOSE OF FAIRVIEW PHYSICIAN ASSOCIATES NETWORK IS TO ADVANCE THE HEALTH OF THE COMMUNITY BY CONTINUOUSLY IMPROVING THE QUALITY OF HEALTH CARE SERVICES DELIVERED TO INDIVIDUALS.

FAIRVIEW PHYSICIAN ASSOCIATES NETWORK (FPA) CURRENTLY IS ACCOUNTABLE FOR PROVIDING A BROAD ARRAY OF HEALTHCARE SERVICES FOR OVER 525,000 INDIVIDUALS THROUGH OUR CONTRACTS WITH PURCHASERS AND PAYERS. ADDITIONALLY, FHN ASSUMES RISK FOR OVER 478,500 MEMBER LIVES IN SHARED SAVINGS INCENTIVES, INCLUDING 100,600 MEMBER LIVES IN DEFINED NETWORK PRODUCTS. ALTHOUGH MANY OF FPA'S HEALTH IMPROVEMENT AND POPULATION MANAGEMENT INITIATIVES HAVE BEEN DEVELOPED TO INCREASE QUALITY, IMPROVE THE PATIENT EXPERIENCE, AND DECREASE COST TO THE INDIVIDUALS SERVED THROUGH OUR SHARED SAVINGS CARE SYSTEM CONTRACTS, MANY OF THE BENEFITS ACCRUE TO THE ENTIRE POPULATION SERVED THROUGH FPA AFFILIATED CLINICS AND FAIRVIEW HOSPITALS (CLINICAL INTEGRATION).
