DAKOTA'S ADULTS, INC (HEREAFTER DAKOTA ADULTS) PROVIDES RESIDENTIAL SUPPORTS TO 12 PEOPLE WHO HAVE A VARIETY OF INTELLECTUAL, DEVELOPMENTAL AND PHYSICAL DISABILITIES. LIVING WELL DISABILITY SERVICES, WHICH MANAGES DAKOTA ADULTS, IS RECOGNIZED AS A HIGH-QUALITY DISABILITY SERVICE PROVIDER ESPECIALLY FOR PEOPLE WHO HAVE COMPLEX MEDICAL CONDITIONS OTHERWISE DIFFICULT TO SERVE IN TYPICAL RESIDENTIAL OR COMMUNITY SETTINGS. LIVING WELL DISABILITY SERVICES OFFERS A ROBUST WELLNESS PROGRAM TAILORED TO THE INTERESTS AND REQUIREMENTS OF EACH PERSON.(CONTINUED ON SCHEDULE O)(CONTINUED FROM PART III)RESIDENTIAL: STARTED BY FAMILIES AS A CHILDREN'S HOME (DAKOTA'S CHILDREN), LIVING WELL DISABILITY SERVICES CONTINUES TO SET THE GOLD STANDARD FOR 24/7 RESIDENTIAL SUPPORTS FOR PEOPLE WITH INTELLECTUAL, PHYSICAL, AND DEVELOPMENTAL DISABILITIES. DAKOTA ADULTS WAS BUILT IN 1981 TO ACCOMMODATE THE FIRST GROUP OF CHILDREN WHO WERE NOW TURNING 22. DAKOTA ADULTS WAS FUNDED WITH A HOUSING AND URBAN DEVELOPMENT (HUD) MORTGAGE. BECAUSE OF THE HUD FUNDING, THIS HOME WAS SEPARATELY INCORPORATED TO COMPLY WITH HUD'S REQUIREMENTS. LIVING WELL DISABILITY SERVICES IS LEVERAGING TECHNOLOGY TO BETTER SUPPORT INDEPENDENCE FOR PEOPLE WHOM WE SERVE.WELL TRAINED, FULL- AND PART-TIME STAFF PROVIDE EXCEPTIONAL INDIVIDUALIZED SUPPORT SO THAT EACH PERSON CAN LIVE WELL. LIVING WELL DISABILITY SERVICES SERVES A BROAD SPECTRUM OF PEOPLE WITH INTELLECTUAL, DEVELOPMENTAL, AND PHYSICAL DISABILITIES SUCH AS CEREBRAL PALSY, DOWN SYNDROME, MULTIPLE SCLEROSIS, AUTISM, AND BRAIN INJURY. THESE MAY BE COMPLICATED BY HEALTH ISSUES RELATED TO AGING, DIABETES, ALZHEIMER'S, AND MEDICAL FRAGILITY. WE SUPPORT PEOPLE WHO HAVE DUAL DIAGNOSES OF INTELLECTUAL DISABILITIES AND MENTAL HEALTH.
