KISHHEALTH SYSTEM HOME CARE DELIVERS QUALITY HOME HEALTH CARE TO DEKALB COUNTY AND NORTHERN ILLINOIS AREA RESIDENTS WHO PREFER TO RECEIVE CARE IN THE COMFORT OF THEIR HOME. WITH THE BACKING OF THE HEALTH SYSTEM, THE TRANSITION FROM HOSPITAL TO HOME IS SIMPLE AND UNCOMPLICATED. EACH PATIENT'S CARE IS MANAGED BY A REGISTERED NURSE, AND OUR TEAM OF REGISTERED NURSES, HOME HEALTH AIDES, THERAPISTS, AND SOCIAL WORKERS WORK TOGETHER TO BRING OUR PATIENTS EXTRAORDINARY CARE IN THE COMFORT OF THEIR OWN HOMES. OUR TEAM APPROACH TO CARE SUPPORTS THE PHYSICAL AND EMOTIONAL NEEDS OF THE PATIENT, CAREGIVERS AND LOVED ONES. WE WORK WITH THE PATIENT, FAMILY AND PHYSICIANS TO SUPPORT THE CARE PLAN AND PROVIDE SERVICES THAT MEET EACH PERSON'S INDIVIDUALIZED NEEDS.
