SHAWNEE ALLIANCE FOR SENIORS PROGRAMS - SHAWNEE ALLIANCE CARE COORDINATORS HELP OLDER ADULTS AND THEIR CAREGIVERS CONNECT TO SERVICES AVAILABLE IN THE COMMUNITY WHICH ENABLE THEM TO MAINTAIN THEIR INDEPENDENCE. CARE COORDINATORS MAKE HOME VISITS TO EVALUATE THE OLDER ADULTS' ABILITY TO FUNCTION INDEPENDENTLY.
