MEDICAL RESPITE PROGRAM PROVIDES 64 BEDS FOR MEN AND WOMEN WHO ARE HOMELESS AND REFERRED BY AREA HOSPITALS WHO NEED A PLACE TO RECOVER FROM INJURY AND ILLNESS FOR SHORT TERM PERIODS. RESIDENTS ALSO RECEIVE ACCESS TO HEALTHCARE AND SOCIAL SERVICES. 91% OF THE RESIDENTS COMPLETE THEIR MEDICAL RECOVERY PLANS. 80% OF THE RESIDENTS FROM THE PROGRAM WERE SUCCESSFULLY DISCHARGED TO APPROPRIATE HOUSING AND TREATMENT. 139 INDIVIDUALS SERVED.
HOUSING PARTNERING WITH THE CITY OF CHICAGO AND ALL CHICAGO, WE HELP 120 HOMELESS INDIVIDUALS AND THEIR FAMILIES ANNUALLY TO SECURE HOUSING THROUGH RAPID RE-HOUSING AND THE EXPEDITED HOUSING INITIATIVE. OUR HEALTH AND HOUSING OUTREACH TEAM PROVIDES A CONTINUUM OF CARE THROUGH HOME VISITS, COUNSELING AND SUPPORT FOR INDIVIDUALS PLACED IN INDEPENDENT HOUSING. THIS INCLUDES SERVING RESIDENTS OF THE LAKE STREET STUDIOS, A COLLABORATIVE HOUSING FACILITY OF THE INTERFAITH DEVELOPMENT CORPORATION, AND THE CHICAGO HOUSING AUTHORITY.
BEHAVIORAL HEALTH: OUR BEHAVIORAL HEALTH PROFESSIONALS PROVIDE SUBSTANCE ABUSE, INDIVIDUAL AND GROUP SERVICES TO OUR CLIENTELE ACROSS ALL PROGRAMS. IN PARTNERSHIP WITH PCC COMMUNITY WELLNESS CENTER, WE ALSO PROVIDE ADDITIONAL BEHAVIORAL HEALTH SERVICES AT OUR ONSITE CLINIC OR THROUGH OUR ONLINE TELEHEALTH SYSTEM. WORKFORCE DEVELOPMENT: WE HAVE A COMPREHENSIVE, CLIENT-CENTERED WORKFORCE TRAINING MODEL THAT INFUSES CURRICULUM, RESUME CREATION, JOB SEARCH AND PLACEMENT SERVICES IDENTIFY OPPORTUNITIES FOR OUR CLIENTS WHO WISH TO FIND JOBS OR VOLUNTEER IN THE COMMUNITY OR AT THE AGENCY. IN ADDITION TO GUIDING CLIENTS TO EMPLOYMENT, WE ARE PROVIDING THEM WITH SOFT SKILLS AND JOB READINESS TRAINING TO HELP THEM BECOME AS READY AS POSSIBLE FOR THE WORKFORCE.
