TO SUPPORT THE MISSION AND MINISTRY OF THE SISTERS OF CHARITY OF ST. AUGUSTINE, JOSEPH'S HOME PROVIDES MEDICAL RESPITE TO HOMELESS ADULTS THROUGH TEMPORARY EMERGENCY SHELTER AND ASSISTS THEM IN ACHIEVING MEDICAL STABILITY AND PERMANENT HOUSING.

JOSEPH'S HOME WAS ESTABLISHED IN 2000 AS A MINISTRY OF THE SISTERS OF CHARITY OF ST. AUGUSTINE. IT IS THE ONLY MEDICAL RESPITE PROGRAM SERVING PEOPLE EXPERIENCING HOMELESSNESS IN NORTHEAST OHIO. ADULTS 18 AND OVER EXPERIENCING HOMELESSNESS AND ACUTE MEDICAL ISSUES ARE REFERRED THROUGH CUYAHOGA COUNTY'S CENTRALIZED COORDINATED INTAKE. JOSEPH'S HOME 24-HOUR INTERDISCIPLINARY TEAM AND BOARD LEADERSHIP WORK WITH EACH PERSON THROUGH AN INTEGRATED, CLIENT-CENTERED APPROACH TO INCREASE MEDICAL, ECONOMIC AND HOUSING STABILITY. POST-DISCHARGE SERVICES CONTINUE WITH CARE COORDINATION AND PEER SUPPORT TO ENSURE THAT CLIENTS MAINTAIN LONG-TERM HEALTH AND HOUSING STABILITY. IN 2021, 48 MEN WERE RESIDENTS OF JOSEPH'S HOME.
