FAMILY OUTREACH SERVICES - A "WRAP-AROUND" PROGRAM FOR INDIVIDUALS/FAMILIES WHO REQUIRE A MULTITUDE OF SERVICES LINKING NEEDS TO OTHER COMMUNITY RESOURCES WHILE PROVIDING LIMITED AMOUNTS OF FINANCIAL ASSISTANCE.
SENIOR SERVICES - HELP FAMILIES AND THEIR CAREGIVERS NAVIGATE ISSUES ASSOCIATED WITH AGING. SERVICES INCLUDE IN-HOME ASSESSMENTS, INDIVIDUALIZED SERVICE PLANS, WELL-BEING CHECKS, FRIENDLY VISTOR, POST-DISCHARGE TREATMENT AND FOLLOW-UP, COUNSELING AND OTHER FAMILY SUPPORT.
