PREMIER HEALTH WAS A SUBWARDEE FOR FUNDING FROM THE OHIO DEPARTMENT OF HEALTH FOR THE COMPREHENSIVE CARE FOR SUBSTANCE USE IN OHIO EMERGENCY DEPARTMENTS (CCOED) GRANT. THE CCOED GRANT HAS THREE OVERARCHING GOALS FOCUSED ON TREATING SUBSTANCE USE DISORDER AND OPIOID USE DISORDER:1) IDENTIFYING PATIENTS WITH OPIOID USE DISORDER BY IMPLEMENTING A SCREENING PROCESS IN OUR EMERGENCY DEPARTMENTS.2) MANAGING OPIOID USE DISORDER BY IMPLEMENTING EVIDENCE-BASED PRACTICES IN OUR EMERGENCY MEDICINE.3) TRANSITIONING PATIENTS TO LONG-TERM CARE AND SUPPORTIVE SERVICES USING INNOVATIVE PROCESSES THAT IMPROVE PATHWAYS TO TREATMENT.PREMIER HEALTH HAS IMPLEMENTED SEVERAL INNOVATIVE APPROACHES FOR TREATING SUBSTANCE USE DISORDER AND ADDICTION MEDICINE WITH THIS GRANT. PREMIER HEALTH WAS THE FIRST IN THE MIAMI VALLEY REGION TO HIRE A SUBSTANCE USE NAVIGATOR (SUN). THE SUN WORKS WITH PATIENTS IN THE EMERGENCY DEPARTMENT TO COMPLETE CHEMICAL DEPENDENCY ASSESSMENTS AND PROVIDE LINKAGES TO CARE, PLACEMENT IN TREATMENT FACILITIES, AND OTHER WRAPAROUND SERVICES WITH WARM HANDOFFS.IN 2022, OVER 30,000 PATIENTS WERE SCREENED IN THE MIAMI VALLEY HOSPITAL EMERGENCY DEPARTMENT FOR SUBSTANCE USE DISORDER. SINCE AUGUST 2022, OVER 400 PATIENTS HAVE BEEN TREATED BY SUBSTANCE USE NAVIGATORS, AND THE PROGRAM DISTRIBUTED OVER 200 NALOXONE KITS.
PROMISE TO HOPE IS A DONOR-FUNDED SERVICE OFFERED AT MIAMI VALLEY HOSPITAL FOR PREGNANT AND POSTPARTUM WOMEN WITH SUBSTANCE USE DISORDER. THE PROGRAM STAFF COORDINATES PRENATAL AND POSTPARTUM CARE AND CONNECTS PATIENTS WITH CHEMICAL DEPENDENCY TREATMENT AND MENTAL HEALTH SERVICES. PROGRAM STAFF SUPPORTS THE MOTHER AND BABY UNTIL THE INFANT'S FIRST BIRTHDAY. THE TEAM CONDUCTS WEEKLY SUPPORT GROUP MEETINGS TO PROMOTE A SAFE PLACE FOR MOMS TO MEET AND BUILD HEALTHY RELATIONSHIPS, LEARN PARENTING AND LIFE SKILLS, AND CONNECT WITH RESOURCES AVAILABLE IN THE COMMUNITY. OUTSIDE SPEAKERS ATTEND THESE MEETINGS TO PROVIDE EDUCATION AND SHARE INFORMATION ABOUT THEIR SERVICES.PROMISE TO HOPE HAS CULTIVATED STRATEGIC PARTNERSHIPS WITH AREA COMMUNITY SERVICE AGENCIES TO OFFER SEAMLESS ACCESS TO CARE. COLLABORATION WITH OUR PROGRAM PARTNERS HELPS PROVIDE ONGOING SUPPORT AND COORDINATED CARE. THIS CARE INCLUDES MEDICATION-ASSISTED TREATMENT, GROUP AND INDIVIDUAL COUNSELING, PRENATAL CARE, WEEKLY SUPPORT GROUPS, AND OTHER SOCIAL SERVICES. PROMISE TO HOPE CARE COORDINATORS ASSESS EACH MOM'S UNIQUE NEEDS TO PROVIDE CONTINUING INDIVIDUALIZED SUPPORT, ADVOCACY, AND REFERRALS. SINCE ITS INCEPTION IN MAY 2015, 792 MOMS HAVE ENROLLED IN THE PROGRAM. IN 2022, 122 MOTHERS ENROLLED IN PROMISE TO HOPE.
MIAMI VALLEY HOSPITAL FOUNDATION RAISES FUNDS TO SUPPORT THE WORK OF CERTIFIED DIABETES EDUCATORS TO PROVIDE PATIENT EDUCATION. BASED AT THE BULL FAMILY DIABETES CENTER, THE DIABETES EDUCATORS COMPLETED 1,544 PATIENT VISITS IN 2022 FOR UNINSURED AND UNDERINSURED INDIVIDUALS. PROCEEDS FROM THE VALLEY CLASSIC GOLF TOURNAMENT HELP SUSTAIN THIS VITAL PROGRAM THAT ENHANCES THE LIVES OF DIABETES PATIENTS THROUGHOUT OUR REGION.
