SUPPORTS PROVIDED INCLUDED DIRECT SUPPORT IN THE FORM OF DAILY LIVING SUPPORT, JOB COACHING, PHYSICAL ASSISTANCE AND VOCATIONAL TRAINING. PROVIDED TRANSPORTATION TO SELECT ACTIVITIES AND JOB SITES DURING PROGRAM HOURS.
