THE MISSION OF LAKESHORE COMMUNITY SERVICES, INC. (LAKESHORE) IS TO ENHANCE THE ABILITY OF PERSONS WITH MENTAL AND PHYSICAL DISABILITIES TO LIVE AS INDEPENDENTLY AS POSSIBLE THROUGH ACQUISITION OF SKILLS AND UTILIZATION OF SERVICES THAT PROMOTE EACH INDIVIDUAL'S SELF DIRECTION, PERSONAL WORTH AND VALUE.

RESIDENTIAL - PROVIDES 24-HOUR RESIDENTIAL/HABILITATIVE SERVICES THAT PROTECT THE INDIVIDUAL DIGNITY AND HEALTH OF PERSONS WITH INTELLECTUAL DISABILITIES.


SUPPORTED LIVING - PROVIDES SUPPORT SYSTEM THAT TEACHES PERSONS WITH INTELLECTUAL DISABILITIES THE SKILLS NECESSARY TO INCREASE THEIR INDEPENDENCE AND LIVE SUCCESSFULLY IN THE COMMUNITY. FAMILY LIVING - OFFERS INDIVIDUALS WITH ITELLECTUAL DISABILITIES THE OPTION TO SHARE THEIR LIVES WITH MEMBERS OF THE COMMUNITY IN AN EXTRAORDINARY WAY. A FAMILY IS "MATCHED" TO A PERSON SEEKING THE COMFORT AND SUPPORT OF THAT TRADITIONAL SETTING.


MENTAL HEALTH SERVICES - PROVIDES SUPPORT SYSTEMS FOR INDIVIDUALS WITH SERIOUS AND PERSISTENT MENTAL ILLNESS. CASE MANAGERS CONNECT INDIVIDUALS WITH FURTHER SUPPORT PROVIDED BY LAKESHORE, INCLUDING MONITORING PROPER USE OF MEDICATION.


HOME AND COMMUNITY REHABILIATION - THIS PROGRAM IS DIVIDED INTO TWO SUBPROGRAMS. SUPPORTED LIVING SERVES INDIVIDUALS IN THEIR OWN HOME. DAY SUPPORT FOCUSES ON VOLUNTEER ACTIVITIES THAT SIMULATE THE WORK ENVIRONMENT AND TEACHES THE SKILLS NEEDED FOR ADULTS WITH INTELLECTUAL DISABILITIES TO SUCCEED IN EVERY DAY LIFE.
