THE BLENDED CASE MANAGEMENT PROGRAM PROVIDES CASE MANAGEMENT SERVICES TO CHILDREN, ADOLESCENTS, AND ADULTS WITH SERIOUS MENTAL ILLNESS OR EMOTIONAL DISORDER WHO REQUIRE ASSISTANCE WITH DAILY LIVING, HOUSING, EDUCATION, WORK, TREATMENT, AND SOCIALIZATION.
THE ERIE OUTPATIENT CLINIC OPERATES IN ERIE COUNTY SERVING ADULTS WITH SERIOUS MENTAL ILLNESSES. THE PRIMARY FUNCTION OF THE CLINIC IS TO MANAGE MEDICATIONS BUT ALSO OFFERS INDIVIDUAL AND GROUP THERAPIES.
