PARTIAL CARE PROGRAM PROVIDES RECOVERY ORIENTED OUTPATIENT CARE IN A FREESTANDING COMMUNITY BASED SETTING. PROGRAM INCLUDES ILLNESS MANAGEMENT, WELLNESS ACTIVITIES, PERSON CENTERED PLANNING, PSYCHIATRIC CARE AND NURSING, SOCIAL AND RECREATIONAL SERVICES, SUPPORTED EDUCATION AND PREVOCATIONAL ACTIVITIES.
PACT PROGRAMS PROVIDE COMPREHENSIVE, INTEGRATED REHABILITATION TREATMENT AND SUPPORT SERVICES TO THOSE INDIVIDUALS WHO ARE MOST CHALLENGED WITH SERIOUS MENTAL ILLNESS. PACT INCLUDES 24 HOUR 7 DAY A WEEK SUPPORT, MEDICATION SUPPORT AND EDUCATION, CRISIS RESPONSE, MICA TREATMENT, DIRECT ASSISTANCE WITH LIFE TASKS, ASSISTANCE IN DEVELOPING PEER AND COMMUNITY SOCIAL SUPPORTS,CAREER COUNSELING.
