THE CDC HEALTH EQUITY GRANT: TO IMPLEMENT HEALTH EQUITY ACTIVITIES, INCLUDING SUPPORTING HEALTHCARE FACILITIES TO DEVELOP COMMUNITY HEALTH NEEDS ASSESSMENT AND IMPLEMENTATION PLANS, FACILITATING REGIONAL HEALTH EQUITY TASK FORCES, AND PROVIDING HEALTH EQUITY EDUCATION. ADDITIONALLY, THE GRANT FINANCIALLY SUPPORTS SUBCONTRACTS FOR THE DEVELOPMENT OF A MONTANA SPECIFIC SOCIAL DETERMINANTS OF HEALTH TOOL (MONTANA INDEX OF HEALTHY COMMUNITIES) AND REGIONAL CARE COORDINATION TO PROMOTE THE COMMUNITY HEALTH WORKER AND COMMUNITY INTEGRATED HEALTH PROFESSIONAL WORKFORCE.
