AGNES CONTINUING CARE CENTER OPERATES MERCY LIFE, A PACE (PROGRAM OF ALL-INCLUSIVE CARE FOR THE ELDERLY) PROGRAM IN PHILADELPHIA, PA THAT PROVIDES COMPREHENSIVE MEDICAL, HEALTH, RECREATIONAL, AND SOCIAL SERVICES FOR FRAIL ELDERS TO ASSIST THEM TO LIVE INDEPENDENTLY. THE MERCY LIFE PROGRAM SERVES SENIORS WITH A RANGE OF HEALTH CONDITIONS THAT IMPACT THEIR DAILY LIFE SUCH AS CHRONIC ILLNESS AND MEMORY LOSS.
