CARE MANAGERS BUILD TRUST WITH MEMBERS, ENSURING ACCESS TO HEALTHCARE, SCHEDULING APPOINTMENTS, COORDINATING VISITS, AND CONNECTING TO PHARMACIES AND COUNSELORS.
