FOUNDATION FOR HEALTH EQUITY SUPPORTS COMMUNITIES OF NW PHILADELPHIA BY BUILDING ON THE ASSETS OF ORGANIZATIONS THAT WORK TO ADDRESS SYSTEMIC HEALTH INEQUITIES IN ORDER TO BUILD HEALTHY, VIBRANT NEIGHBORHOODS.

THE PURPOSE OF FOUNDATION FOR HEALTH EQUITY IS TO IMPROVE THE HEALTH AND WELL BEING OF COMMUNITIES OF NORTHWEST PHILADELPHIA AND EASTERN MONTGOMERY COUNTY. THROUGH ONGOING NEEDS ASSESSMENTS, IT IDENTIFIES CERTAIN AREAS OF VULNERABILITY IN THESE COMMUNITIES, AND PROVIDES FUNDING TO ORGANIZATIONS WHOSE WORK WILL ADDRESS THESE NEEDS. THE FOUNDATION'S FOCUS IS ON THESE SPECIFIC AREAS: THE FRAIL ELDERLY, REDUCING PREMATURE DEATH CAUSED BY CANCER AND HEART DISEASE, CHILDREN AND FAMILIES, EDUCATION ABOUT HEALTHCARE,HEALTHCARE AND HEALTH INSURANCE OPTIONS. WHERE APPROPRIATE,THE FOUNDATION FAVORS PROGRAMS AIMED AT PREVENTION.
