HEALTH HOME CARE MANAGEMENT - CONSISTS OF PROVIDERS AND COMMUNITY AGENCIES THAT COLLABERATE TO PROVIDE SERVICES TO PEOPLE WITH MEDICAID COVERAGE WHO HAVE A SERIOUS MENTAL ILLNESS OR TWO CHRONIC CONDITIONS. IT ENABLES PATIENTS TO BE HEALTHIER AND SAFER IN THE COMMUNITY.
