HOME HEALTH AIDES - TRAINED PERSONNEL TO PERFORM TASKS DICTATED BY A NURSE TO CARE FOR PATIENTS IN THE PATIENT'S HOME. EXAMPLES ARE BATHING, ASSIST WITH DRESSING, AMBULATION AND EXERCISES DICTATED BY A PHYSICAL THERAPIST. INCLUDES SERVICES TO AFFILIATES.
NURSE-FAMILY PARTNERSHIP PROGRAM, TELEMEDICINE SERVICES, AND MISCELLANEOUS SERVICES.
HEALTH HOME CARE MANAGEMENT - CONSISTS OF PROVIDERS AND COMMUNITY AGENCIES THAT COLLABERATE TO PROVIDE SERVICES TO PEOPLE WITH MEDICAID COVERAGE WHO HAVE A SERIOUS MENTAL ILLNESS OR TWO CHRONIC CONDITIONS. IT ENABLES PATIENTS TO BE HEALTHIER AND SAFER IN THE COMMUNITY.
