THE PLAN PROVIDES REIMBURSEMENT FOR HEALTH RELATED EXPENSES INCURRED BY THE MEMBER, SPOUSE OR ELIGIBLE DEPENDENT INCLUDING, BUT NOT LIMITED TO, CHARGES BY ANY DOCTOR, DENTIST, OPTOMETRIST, OPHTHALOMOLOGIST, HOSPITAL OR OTHER HEALTH FACILITY, PHARMACY, ALLOWABLE OVER-THE-COUNTER DRUGS, OPTICAL DISPENSING SERVICE, OR HEARING AID PROVIDER. IN ADDITION, THE PLAN ALSO ALLOWS A MEMBER TO BE REIMBURSED FOR SELF-PAY HEALTH INSURANCE PREMIUM COSTS IF THEY LOSE COVERAGE THROUGH THE RELATED WELFARE PLAN.
