HOSPICE SERVICES: HOME HEALTH AIDE SERVICES - 9,150 VISITS SOCIAL SERVICES - 1,691 VISITS COUNSELING SERVICES - 909 VISITS THERAPEUTIC SERVICES - 36 VISITS COMPLEMENTARY CARE SERVICES - 618 VISITS SKILLED NURSING SERVICES - 5,732 VISITS BEREAVEMENT SERVICES - 413 ONE-ON-ONE VISITS
THE ANNA AND LOUIS H. SHEREFF CAREGIVER PROGRAM: THE TARGET POPULATION OF THE PROGRAM REPRESENTS INDIVIDUALS WHO ARE OTHERWISE ELIGIBLE FOR ADMISSION TO HOSPICE OF WESTCHESTER BUT LACK A PRIMARY CAREGIVER WHO IS ABLE TO PROVIDE THE NECESSARY CARE; AND, WHO ARE NOT COVERED BY PRIVATE OR GOVERNMENTAL INSURANCE PROGRAMS THAT PAY FOR SUCH CARE; AND, LACK THE FINANCIAL RESOURCES TO PAY FOR A SUBSTITUTE CAREGIVER. IN CIRCUMSTANCES WHERE THE CAREGIVER NEEDS ASSESSMENT FORM IDENTIFIES AREAS OF PATIENT NEED, HOSPICE OF WESTCHESTER WILL ARRANGE FOR AND PAY DIRECTLY FOR AN AGENCY AIDE. IN ADDITION, HOSPICE OF WESTCHESTER WILL ALSO INCLUDE PAYMENT FOR A PERSONAL EMERGENCY RESPONSE UNIT. THE PURPOSE OF THIS PROGRAM IS TO ENABLE FAMILIES TO STAY TOGETHER IN THEIR HOME WHILE ON HOSPICE AND TO AVOID PLACEMENT IN A FACILITY. THE GRANT IS MOST OFTEN USED IN CIRCUMSTANCES WHERE THE CAREGIVER IS NOT WELL ENOUGH TO CARE FOR THE PATIENT. THE GRANT ALLOWS FOR CARE TO BE PROVIDED SO THAT THE PATIENT'S NEEDS ARE ATTENDED TO AND THE CAREGIVER AND PATIENT CAN REMAIN TOGETHER AT HOME. THE CAREGIVER PROGRAM HAS BEEN EXPANDED TO ALSO SUPPORT COMPLEMENTARY CARE SERVICES. THE ORGANIZATION SERVED 59 PATIENTS UNDER BOTH THE CAREGIVER AND COMPLEMENTARY CARE PROGRAMS DURING 2021.
