CCL - COMMUNITY CARE LINK: THIS MODEL MANAGEMENT SERVICES ORGANIZATION, THE FIRST OF ITS KIND, IS A COLLABORATIVE INITIATIVE IMPLEMENTED BY DFTA AND THE AGING IN NEW YORK FUND, IN PARTNERSHIP WITH HEALTHFIRST. THE GOAL IS TO REACH OUT TO A LIST OF ENGLISH AND SPANISH-SPEAKING MEDICARE RECIPIENT MEMBERS IDENTIFIED BY HEALTHFIRST, WHO HAVE NOT CONNECTED WITH A PRIMARY CARE PROVIDER, AND HELP THEM MAKE THE CONNECTION. COMMUNITY NAVIGATORS FROM 5 CBOS: JASA, SELFHELP, SAGE, BAY RIDGE SENIOR CENTER, AND CARTER BURDEN, OUTREACH TO MEMBERS RESIDING WITHIN SPECIFIED ZIP CODES. THE PROGRAM WAS CONCEIVED BEFORE THE PANDEMIC AND WAS REVISED TO BETTER MEET CLIENT NEEDS AS PEOPLE SELF-QUARANTINED AND WERE FORCED TO REMAIN IN THEIR HOMES. EACH AGENCY IS REIMBURSED AT SET RATES FOR THE SERVICES THEY PROVIDE TO CLIENTS, USING PEER PLACE AND STARS TO DOCUMENT EACH CALL.
ENGAGEMENT AND LONGEVITY IN MEDICINE (ELM) IS A 3-YEAR PARTNERSHIP WITH THE AGING IN NEW YORK FUND (FUND), THE NEW YORK CITY DEPARTMENT FOR THE AGING AND NYU LANGONE HEALTH TO BRING AWARENESS TO COMMUNITIES OF COLOR ABOUT THE IMPORTANCE OF CLINICAL TRIALS AND PARTICIPATING IN THEM. ELM EXPLORES DIFFERENT OUTREACH AND ENGAGEMENT STRATEGIES TO INCREASE RECRUITMENT IN RESEARCH OF OLDER ADULTS FROM DIVERSE COMMUNITIES. THROUGH THIS EFFORT, ELM AIMS TO BETTER EDUCATE OLDER ADULTS AND ENGAGE THEM IN CLINICAL RESEARCH AND IN BETTER PERSONAL HEALTH AND WELLNESS OUTCOMES. A GOAL OF THE ELM IS TO CREATE A HEALTH COMMUNITY FOR OLDER ADULTS THROUGH ITS ELECTRONIC REGISTRY. THE FIRST OBJECTIVE WAS TO IDENTIFY THE GREATEST COMMUNITIES OF NEED, THEN RECRUIT AND RETAIN CULTURALLY AND LINGUISTICALLY COMPETENT COMMUNITY NAVIGATORS THAT ARE REPRESENTATIVE OF THE TARGET POPULATION. THROUGH DATA ANALYSIS, IT WAS DETERMINED THAT SEVERAL AREAS OF THE CITY WERE THE MOST DENSELY POPULATED AND UNDER-REPRESENTED IN THESE EFFORTS. TO BEST GAIN ACCESS TO OLDER ADULT CLIENT STAKEHOLDERS, STAFF ESTABLISHED A GROUP OF "THOUGHT LEADERS" FROM CARTER BURDEN, NEIGHBORHOOD SHOPP, JASA, VISIONS, AND PSS. WITH THEIR INPUT, A SERIES OF VIRTUAL HEALTH EDUCATION PROGRAMS WERE OFFERED, ALONG WITH WRITTEN MATERIALS IN SPANISH/ENGLISH ABOUT CLINICAL TRIALS. ADDITIONAL PROGRAMS ARE BEING DEVELOPED TO CREATE SUSTAINABLE RESOURCES ABOUT CLINICAL TRIALS AND RESEARCH.
SUCCESSFUL AGING FOR INCREASED LONGEVITY (SAIL) PROGRAM: THE AGING IN NEW YORK FUND PARTNERED WITH FLUSHING HOSPITAL AND CHINESE-SPEAKING MEMBERS OF SENIOR CENTERS TO HELP REDUCE SOCIAL ISOLATION AND DEPRESSION. ORIGINALLY DESIGNED FOR IN-PERSON OUTREACH, USING TECHNOLOGY, STAFF CONDUCTED VIRTUAL OUTREACH TO CONNECT WITH CHINESE RESIDENTS ADVERSELY AFFECTED BY THE PANDEMIC. CHINESE-SPEAKING OLDER ADULTS WERE HELPED TO OVERCOME FEAR AND STIGMA ASSOCIATED WITH MENTAL HEALTH ISSUES. AS A RESULT, STAFF CONNECTED OLDER ADULT CHINESE-SPEAKING CLIENTS BY PHONE AND OTHER TECHNOLOGY TO CHINESE-SPEAKING THERAPISTS AT FLUSHING HOSPITAL CLINICS FOR COUNSELING AND MEDICATIONS WHERE NECESSARY.
OTHER PROGRAM ACTIVITIES INCLUDING AGE-TASTIC! A HEALTH AND WELLNESS PROGRAM FOR OLDER ADULTS, RELATIVES AS PARENTS PROGRAM (RAPP), AND ASSIGNED HOUSING COUNSEL PROGRAM.
