THE SKILLED NURSING PROGRAM PROVIDES VARIOUS LEVELS OF LONG TERM CARE BASED ON THE INDIVIDUAL'S NEEDS. IT ALSO INCLUDES SHORT TERM SUB-ACUTE REHABILITATION SERVICES DESIGNED FOR ADULTS WHO ARE RECOVERING FROM INJURY, SURGERY OR DISEASE. THE SKILLED NURSING PROGRAM HAS SERVICED 769 ADULTS PROVIDING APPROXIMATELY 44,617 DAYS OF CARE THROUGHOUT 2021.
THE CERTIFIED HOME HEALTH CARE PROGRAM OFFERS IN-HOME CARE FOR PEOPLE LIVING WITH CHRONIC ILLNESS OR DISABILITIES. PROFESSIONAL SERVICE PLANS ARE DEVELOPED FOR EACH INDIVIDUAL BASED ON THEIR NEEDS IN CONSULTATION WITH THE PATIENT, FAMILY AND PERSONAL PHYSICIAN.
