SERVING PEOPLE LIVING WITH DEMENTIA AND FAMILY CARE PARTNERS THROUGH A MULTIDISCIPLINARY BEHAVIORAL AND SOCIAL SUPPORT APPROACH THROUGH: * DEMENTIA NAVIGATION * CAREGIVER COUNSELING * EDUCATION & OUTREACH * OCCUPATIONAL THERAPYSINCE INCEPTION, MEMORY CARE HOME SOLUTIONS (MCHS) HAS IMPACTED OVER 270,000 INDIVIDUALS. USING A FAMILY-CENTERED APPROACH, WE FOCUS ON MODIFYING THE HOME ENVIRONMENT TO SUPPORT THE SAFETY AND FUNCTION OF THE PERSON WITH DEMENTIA, WHILE HELPING CAREGIVERS DEVELOP SKILLS TO ADDRESS DIFFICULT SYMPTOMS AND BEHAVIORS.
