CARE MANAGERS MEET WITH FAMILIES IN THEIR HOMES AND DEVELOP A TEAM OF FORMAL AND INFORMAL SUPPORTS TO IDENTIFY NEEDS AND THE STRATEGIES TO MEET THOSE NEEDS. THE GOAL IS FOR THE FAMILY TO SUCCEED WITHOUT THE NEED FOR FORMAL SERVICES SUCH AS THE CMO. FUNDS INCLUDE THE PROVISION OF THE FOLLOWING SERVICES TO YOUTH AND FAMILIES THAT ARE NOT COVERED BY MEDICAID: RENTAL ASSISTANCE, UTILITY ASSISTANCE, MENTORING, CAMP, PSYCHIATRIC ASSESSMENTS, PURCHASING ITEMS SUCH AS FOOD, CLOTHING AND FAMILY ACTIVITY COSTS, ETC.
