WITH THE SMART MODELS GROWTH AND LEVEL OF SUCCESS IN ADDRESSING SYSTEM GAPS AND STRUCTURAL BARRIERS TO HEALTH EQUITY, SMART HAS EXPANDED AND ADAPTED TO A SERVE ADDITIONAL POPULATION COHORTS THROUGH NEW PARTNERSHIPS INCLUDING CHARTER SCHOOLS, ADULT LEARNERS, AND SENIOR TOWER RESIDENTS.
