THE RURAL HEALTH INITIATIVE (RHI) IS A BRIDGE TO IMPROVE AND SUSTAIN THE HEALTH AND SAFETY OF FARM FAMILIES. RHI MAKES HOME VISITS TO FARM FAMILIES WHERE OUTREACH HEALTH COORDINATORS PERFORM HEALTH RISK ASSESSMENTS, BLOOD PRESSURE AND CHOLESTEROL SCREENINGS, AND MAKES REFERRALS FOR HEALTH CARE PROVIDERS AND SOCIAL SERVICES AT NO COST TO THE FARM FAMILIES.
