THE MISSION OF HOPE HOSPICE, INC. IS TO PROVIDE HIGH QUALITY MEDICAL CARE AND EMOTIONAL SUPPORT FOR TERMINALLY ILL PATIENTS AND THEIR FAMILIES/CAREGIVERS.

PATIENT CARE PROGRAM - HOPE HOSPICE PROVIDES MEDICAL CARE, SOCIAL SERVICES, AND FAMILY SUPPORT FOR PATIENTS WITH A DIAGNOSIS OF SIX MONTHS OR LESS TO LIVE. THE CLINICAL TEAM INCLUDES HOSPICE PHYSICIANS, REGISTERED NURSES, LICENSED VOCATIONAL NURSES, HOME HEALTH AIDS,SOCIAL WORKERS, SPIRITUAL CARE COUNSELORS, AND VOLUNTEERS. MEDICAL CARE INCLUDES PAIN MANAGEMENT, PSYCHOSOCIAL CONCERNS, SPIRITUAL CARE, AND EDUCATION FOR FAMILY CAREGIVERS.HOPE HOSPICE ALSO PROVIDES GRIEF SUPPORT AND EDUCATION FOR THE FAMILY/CAREGIVER FOR 13 MONTHS FOLLOWING THE DEATH OF A PATIENT. SUPPORT GROUPS AND SERVICES FOR CHILDREN AND TEENS WHO HAVE SUFFERED A LOSS IN THEIR FAMILY ARE ALSO AVAILABLE.
MEDICAL AND COMMUNITY OUTREACH PROGRAM - HOPE HOSPICE PROVIDES COMMUNITY AND PROFESSIONAL EDUCATION PRESENTATIONS TO FAMILIES, MEDICAL PROFESSIONALS, AND MEMBERS OF THE GENERAL PUBLIC. SEMINARS AND WORKSHOPS ARE OFFERED IN HOSPICE CARE, ADVANCE CARE PLANNING, DEMENTIA, AND RELATED END-OF-LIFE CONCERNS. PRESENTATIONS ARE MADE AT COMMUNITY ORGANIZATIONS, LOCAL BOARD AND CARE HOMES, SKILLED NURSING FACILITIES, OTHER MEDICAL CARE PROVIDERS, AND AT HOPE HOSPICE OFFICES AND BUSINESSES. HOPE HOSPICE ALSO PROVIDES VALUABLE TRAINING FOR HOSPICE VOLUNTEERS WHO SUPPORT THE SERVICES WE OFFER. VOLUNTEERS ARE KEY COMPONENT OF HOSPICE CARE; THUS, RECRUITMENT, RETENTION, AND TRAINING OF VOLUNTEERS IS CRITICAL. THE GRIEF SUPPORT ALSO HOST A VARIETY OF GRIEF AND LOSS WORKSHOPS FOR THE COMMUNITY.
