HOSPICE PROVIDES COMPASSIONATE COMFORT CARE - MEDICAL CARE, PAIN MANAGEMENT, EMOTIONAL AND SPIRITUAL SUPPORT - TO PATIENTS AT THE END OF LIFE. THIS PROGRAM IS DESIGNED FOR PATIENTS WITH A TERMINAL ILLNESS OF SIX MONTHS OR LESS, WHO WISH TO MAINTAIN QUALITY OF LIFE. HOSPICE IS CHOSEN BY THE PATIENT WITH A DOCTOR'S CONSENT, AND IS COVERED BY MEDICARE, MEDI-CAL OR PRIVATE INSURANCE.APPROX. 2,457 PATIENTS BENEFITED FROM THE SERVICES IN YEAR 2020.
TRANSITIONS PROVIDES EMOTIONAL SUPPORT AND NURSE CONSULTATION FOR PATIENTS WHO DO NOT MEDICALLY QUALIFY FOR EITHER ADVANCED CARE OR HOSPICE.
