CASE MANAGEMENT PROGRAM: THE CENTER PROVIDED INDIVIDUALIZED CASE MANAGEMENT SERVICES FOR LOW-INCOME CLIENTS MONTHLY THROUGH FUNDING SUPPORT FROM CITY OF NEW ORLEANS COMMUNITY DEVELOPMENT BLOCK GRANT CDBG. SERVICES INCLUDE TRACKING ADLS, IADLS, RISK FACTORS, AND HEALTH STATUS, AND COORDINATION WITH CAREGIVER SUPPORTS, REFERRALS FOR COUNSELING, RESOURCES AND REFERRALS FOR NUTRITION, MEDICAL, PHARMACY NEEDS, AND REFERRALS FOR CAREGIVER SERVICES, LEGAL ASSISTANCE, AND HOME REPAIR.
GENERAL HEALTH & WELLNESS PROGRAM: THE KINSHIP CENTER PROVIDED OVER 1,250 HOURS OF HEALTH AND WELLNESS OPPORTUNITIES TO OVER 130 SENIOR ADULTS AGED 60+. ACTIVITIES THROUGH THIS PROGRAM SERVICE INCLUDE: DAILY EXERCISE, HOT MEALS AS A CONGREGATE MEAL SITE, THERAPEUTIC ARTS, MUSIC, ACTIVITY CLUBS, FIELD TRIPS.
TRANSPORTATION PROGRAM: THE CENTER PROVIDED OVER 300 PERSON-TRIPS MONTHLY 3,600 TRIPS ANNUALLY TO OVER 40 SENIORS TO BRING THEM TO/FROM THE CENTER, ON FIELD TRIPS, ON EEKLY GROCERY STORE TRIPS, MONTHLY SHOPPING TRIPS TO WALMART AND DOLLAR STORES. FUNDING SOURCES INCLUDE INDIVIDUAL DONATIONS, ST. CHARLES AVENUE PRESBYTERIAN CHURCH AND NEW ORLEANS COUNCIL ON AGING.
