PRIMARY PATIENT CARE MODEL INCLUDES INTEGRATED ACCESS TO MEDICAL, PHARMACY, BEHAVIORAL HEALTH INCLUDING SUBSTANCE USE DISORDERS, AND ORAL HEALTH CARE SERVICES IN COMMUNITIES WHERE ECONOMIC, GEOGRAPHIC, OR CULTURAL BARRIERS LIMIT ACCESS TO AFFORDABLE HEALTH CARE SERVICES. CDHC PROVIDES CARE FOR THE MOST VULNERABLE INDIVIDUALS AND FAMILIES, INCLUDING THOSE EXPERIENCING HOMELESSNESS, RESIDENTS OF PUBLIC HOUSING, RECENTLY INCARCERATED AND VETERANS. MATERNAL AND CHILD HEALTH SUPPORT THROUGH THE OMAHA HEALTHY START, WHICH AIMS TO REDUCE THE RATE OF INFANT MORTALITY IN DOUGLAS COUNTY, WITH SPECIAL ATTENTION ON LOWERING THE NUMBER OF INFANT DEATH RATES THAT OCCUR AMONG AFRICAN-AMERICAN FAMILIES. THE WOMEN, INFANT, AND CHILDREN (WIC) CLINIC LOCATIONS IN DOUGLAS COUNTY, PROVIDING WHOLESOME FOODS, NUTRITION EDUCATION, AND COMMUNITY SUPPORT FOR INCOME-ELIGIBLE WOMEN WHO ARE PREGNANT OR POSTPARTUM, INFANTS, AND CHILDREN UP TO FIVE YEARS OLD FOR AN AVERAGE OF 3,600 FAMILIES PER MONTH.
