CASE MANAGEMENT - THE ORGANIZATION PROVIDES CASE MANAGEMENT SERVICES TO PERSONS WITH PROLONGED AND SEVERE MENTAL ILLNESS, INCLUDING DEVELOPMENTAL DELAYS AND BRAIN INJURIES, TO ASSIST IN COMMUNITY INTEGRATION RESULTING IN STRENGTHENING FAMILY AND NATURAL SUPPORTS, INCREASED COMMUNITY SAFETY, AND INCREASED EMPLOYABILITY.
BEHAVIORAL HEALTH HOMES - A BEHAVIORAL HEALTH HOME (BHH) IS NOT A PLACE OR THING, AND IS INSTEAD A SERVICE DESIGNED TO SERVE PERSONS WHO ARE PRIOR AUTHORIZED BY MAINECARE TO HAVE THEIR BEHAVIORAL HEALTH AND PHYSICAL HEALTH CONDITIONS MANAGED AND COORDINATED BY HOME CARE COORDINATORS AND/ OR CASE MANAGERS, SITUATED IN THE BHH AND PRIMARY CARE HEALTH HOMES (HH). COORDINATORS INTEGRATE AND COORDINATE WITH THE CLIENT'S PRIMARY CARE HH, ALL PRIMARY, ACUTE, BHH, AND LONG TERM SERVICES AND SUPPORTS. A COMPREHENSIVE PLAN OF CARE INCORPORATES BOTH BEHAVIORAL HEALTH AND PRIMARY CARE INFORMATION AND CARE, WHICH RESULTS IN IMPROVED BEHAVIORAL HEALTH AND PRIMARY CARE OUTCOMES FOR MEMBERS.
MEDICATION MANAGEMENT - A GROUP OF MEDICAL AND PSYCHIATRIC SERVICES DESIGNED TO OPTIMIZE TREATMENT FOR PATIENTS, REQUIRING INTEGRATION OF A BROAD RANGE OF PROFESSIONAL ACTIVITIES, INCLUDING, BUT NOT LIMITED TO, PATIENT PSYCHIATRIC AND MEDICAL ASSESSMENT AND/OR A COMPREHENSIVE MEDICATION REVIEW, FORMULATING PSYCHIATRIC PLAN OF CARE, MEDICATION THERAPY, AND COORDINATION OF CARE.
COUNSELING - LICENSED PSYCHOTHERAPISTS TREAT A WIDE RANGE OF BEHAVIORAL PROBLEMS INCLUDING TRAUMA, MAJOR DEPRESSION, GRIEF AND LOSS, SUBSTANCE ABUSE, PERSONALITY DISORDERS, FAMILY PROBLEMS ETC. PSYCHOTHERAPISTS ASSIST TO REDUCE DEPENDENCE AND IMPROVE EMPLOYABILITY BY WORKING THROUGH CASE MANAGEMENT, COORDINATION OF CARE AND SOCIAL SERVICES HUBS, AND ADVANCEMENTS IN TECHNOLOGY. REHABILITATIVE AND COMMUNITY SUPPORT SERVICES FOR CHILDREN WITH COGNITIVE IMPAIRMENTS AND FUNCTIONAL LIMITATIONS (RCS) (SECTION 28) - THE RCS SERVICE IS A DIRECT CARE SERVICE PROVIDED IN THE HOME AND/OR COMMUNITY IN EITHER INDIVIDUAL OR GROUP SETTINGS. THESE SERVICES ARE DESIGNED TO RETAIN OR IMPROVE FUNCTIONAL ABILITIES OF CHILDREN WHICH HAVE BEEN NEGATIVELY IMPACTED BY THE EFFECTS OF COGNITIVE OR FUNCTIONAL IMPAIRMENT AND ARE FOCUSED ON BEHAVIOR MODIFICATION AND MANAGEMENT, SOCIAL DEVELOPMENT, AND ACQUISITION AND RETENTION OF DEVELOPMENTALLY APPROPRIATE SKILLS. THESE SERVICES ARE FOR CHILDREN OR YOUTH WHO HAVE A DEVELOPMENTAL DISABILITY THAT EFFECTS THEIR EVERYDAY LIVING. EXAMPLES INCLUDE AUTISM, SPECTRUM DISORDERS AND INTELLECTUAL DISABILITY. SERVICES INCLUDE SKILL BUILDING IN ACTIVITIES OF DAILY LIVING AND BEHAVIOR MANAGEMENT. DAILY LIVING SUPPORT - THIS IS AN ADULT PROGRAM DESIGNED TO HELP INDIVIDUALS MAINTAIN THE HIGHEST LEVEL OF INDEPENDENCE POSSIBLE THROUGH SUPERVISION AND THERAPEAUTIC SUPPORT. THE GOAL OF THE PROGRAM IS TO HELP INDIVIDUALS WITH THE TASKS OF DAILY LIVING THAT OCCUR BOTH AT HOME AND IN THE COMMUNITY THROUGH COACHING, CUING AND MODELING.
