THE ORGANIZATION IS CREATED TO PROVIDE FUNDS FOR THE PAYMENT OF HEALTH CARE BENEFITS TO EMPLOYEES OF PIONEER PRODUCTS, INC., A WISCONSIN BUSINESS CORPORATION. BENEFITS TO BE PROVIDED INCLUDE GROUP TERM LIFE INSURANCE, ACCIDENTAL DEATH AND DISMEMBERMENT INSURANCE, SHORT-TERM DISABILITY AND MEDICAL BENEFITS. ALL OF THESE BENEFITS ARE DESCRIBED IN THE "SUMMARY PLAN DESCRIPTION" DOCUMENTS OF THE ORGANIZATION. THE ORGANIZATION WILL PAY THE PREMIUMS FOR THE INSURED PORTION OF THE PLAN, AND WILL DELIVER PAYMENT DIRECTLY TO MEMBERS UNDER THE SELF-INSURED PORTION OF THE PLAN.
