THE TRANSPORTATION PROGRAM ASSURES THAT TRANSPORTATION IS AVAILABLE FOR PATIENTS WITH MEDICAL APPOINTMENTS AT LOCAL HEALTH FACILITIES AND SERVES RESIDENTS OF WASHINGTON COUNTY WISCONSIN, THE PROGRAM PROVIDES TRANSPORTATION AT A SUBSIDIZED RATE.
SUPPORT AN ADULT DAY CARE SERVICES FACILITY AND PROGRAM DESIGNED TO PROMOTE INDEPENDENCE IN THE ELDERLY AND DETER THE NEED FOR NURSING HOME PLACEMENT. THE PROGRAM CURRENTLY SERVES ON AVERAGE 15 FAMILIES FROM WASHINGTON COUNTY. DONATIONS HELP UNDERWRITE SCHOLARSHIPS FOR CLIENTS PARTICIPATING IN THE PROGRAM.
