THE ORGANIZATION PROVIDES HOME HEALTH AND HOSPICE CARE TO THE RESIDENTS OF KOSCIUSKO COUNTY. THROUGH THE DIRECTION OF THE PATIENT'S PHYSICIAN, THE ORGANIZATION PROVIDED PROFESSIONAL HEALTH SERVICES TO 241 PATIENTS CONSISTING OF 5,891 IN-HOME VISITS BY NURSES, THERAPISTS, HOME HEALTH AIDES, AND HOMEMAKERS, ALLOWING THE PATIENT TO STAY AT HOME WHILE THEY RECOVER FROM MEDICAL TREATMENT. THE ORGANIZATION ALSO PROVIDED END OF LIFE CARE TO 148 PATIENTS CONSISTING OF 8,825 DAYS OF SERVICE. SUCH SERVICES, PROVIDED BY NURSES, SOCIAL WORKERS, AND HOME HEALTH AIDES, FOCUSES ON PAIN AND SYMPTON CONTROL, AND INCLUDE THE PROVISION OF MEDICATION, SUPPLIES AND MEDICAL EQUIPMENT AS WELL AS INSTRUCTION, SUPPORT, RESPITE SERVICES AND BEREAVEMENT CARE.
THE MEDICATION AND DENTAL ASSISTANCE DIVISION OF OUR ORGANIZATION PROVIDES PAYMENT VOUCHERS FOR PHYSICIAN-PRESCRIBED MEDICATIONS AND EMERGENCY DENTAL SERVICES TO PERSONS IN NEED RESIDING IN KOSCIUSKO COUNTY. IN 2021, VOUCHERS WERE PROVIDED TO INDIVIDUALS IN NEED OF MEDICATIONS, DENTAL CARE, OR MEDICAL SUPPLIES. THESE ACTIVITES ARE FULLY FUNDED BY THE K21 HEALTH FOUNDATION AND KOSICUSKO COUNTY COMMUNITY FOUNDATION.
