CONTRACTS PHYSICIANS TO DIAGNOSIS AND TREAT INDIVIDUALS WITH OPIOID ADDICTION WITH VARIOUS MEDICATIONS SUCH AS VIVITROL,SUBOXONE AND OTHER MEDICATIONS. THE REQUIREMENTS FOR REMAINING ON MEDICATED ASSISTED TREATMENT INCLUDE A FULL SUBSTANCE ABUSE ASSESSMENT, INDIVIDUAL AND GROUP COUNSELING, CASEMANAGEMENT AND TREATMENT PLANNING.INDIVIDUAL AND GROUP COUNSELING, CASEMANAGEMENT AND TREATMENT PLANNING.
