THE PRIMARY PURPOSE OR MISSION IS TO PROVIDE A LEADERSHIP ROLE IN BUCKS COUNTY THROUGH A COLLABORATIVE APPROACH TO IMPROVE THE HEALTH STATUS OF THE COMMUNITY. BCHIP TAKES A COMPREHENSIVE APPROACH TO ADDRESSING THE PROBLEM OF HEALTHCARE ACCESS FOR LOW-INCOME, UNINSUED INDIVIDUALS AND FAMILIES THROUGH THE FOLLOWING: 1)IDENTIFY AND UNDERSTAND NEED AT THE COMMUNITY LEVEL, 2)DELIVER HEALTH PROMOTION AND PREVENTION PROGRAMS THAT HELP ENABLE THE RESIDENTS OF BUCKS COUNTY TO LIVE HEALTHIER LIVES, 3)CONVENE MULTIPLE ORGANIZATIONS THAT CARE ABOUT BUCKS COUNTY TO ADDRESS THE NEEDS OF ITS MOST VULNERABLE RESIDENTS. BCHIP HAD IDENTIFIED THE REDUCTION OF CARDIOVASCULAR DISEASE THROUGH TOBACCO EDUCATION AND CESSATION PROGRAMS COUNTYWIDE AS ONE OF ITS STRATEGIC PRIORITIES. USING MONEY FROM TOBACCO CESSATION GRANTS FROM THE PA DEPARTMENT OF HEALTH AND THE HEALTH PROMOTION COUNCIL, BCHIP HELD SMOKING CESSATION CLASSES THROUGHOUT BUCKS COUNTY. ALL CLASSES WERE PROVIDED FREE OF CHARGE TO PARTICIPANTS WHO ALSO RECEIVED FREE NICOTINE REPLACEMNT PATCHES, GUM OR LOZENGES TO ASSIST THEM IN THEIR ATTEMPT TO QUIT SMOKING.
ADVANCED CARE PLANNING PROGRAM - BCHIP PROVIDES A FREE AND CONFIDENTIAL ADVANCE CARE PLANNING SERVICE TO HELP ADULTS START CONVERSATIONS WITH LOVED ONES ABOUT HEALTHCARE CHOICES LEADING TO A COMPLETION OF AN ADVANCED HEALTHCARE DIRECTIVE TO DOCUMENT CHOICES.
