GRANT PAYMENTS ARE MADE IN COMPLIANCE WITH THE LAST WILL AND TESTAMENTOF ROBERT R RHODES DATED NOVEMBER 4, 1913 AND ALIGNED WITH UPDATED EXHIBIT A NAMED BENEFICIARIES DATED APRIL 19, 1940.KEYBANK, AS TRUSTEE, MAKES DISTRIBUTIONS TO PRE SELECTED EXEMPT ORGANIZATIONS TO BE USED FOR THEIR EXEMPT PURPOSE. KEYBANK MAINTAINS A RESPONSIVENESS TO THE NEEDS AND ACTIVITIES OF EACH BENEFICIARY ORGANIZATION IN THE ATTAINMENT OF ITS MISSION. UNDER THE OHIO INSTITUTIONAL TRUST FUNDS ACT OF 1999, ALL BENEFICIARIES EXCEPT HIRAMHOUSE HAVE CHOSEN TO RECEIVE THE MAXIMUM OF INCOME BASED ON THEGREATER OF 5% OF AFV OR TRADITIONAL INCOME.HIRAM HOUSE HAS CHOSEN TRADITIONAL INCOME. THE TRUST CAN BE TERMINATED BY ANY 25% COMBINATION OF BENEFICIARY INCOME INTERESTS.

MISSION:

PROMEDICA CONTINUING CARE SERVICES CORPORATION PROVIDES A VARIETY OF NON-ACUTE HEALTH SERVICES TO ALL INDIVIDUALS WITHIN THE COMMUNITY. THESE SERVICES PRIMARILY CONSIST OF RESIDENTIAL SKILLED NURSING AND LONG-TERM CARE, AS WELL AS HOME HEALTH AND INPATIENT/OUTPATIENT HOSPICE.

THE FOUNDATION WILL SERVE AS A SUPPORTING ORGANIZATION OF GREATER CINCINNATI BEHAVIORAL HEALTH SERVICES, WHICH IS AN OHIO NONPROFIT CORPORATION, A CODE SECTION 501(C)(3) ORGANIZATION AND A PUBLIC CHARITY DESCRIBED IN CODE SECTION 509(A)(1). IN ITS SUPPORT ROLE, THE FOUNDATION PLANS TO RAISE FUNDS THROUGH CONTRIBUTIONS AND PROVIDE FINANCIAL AND OTHER SUPPORT FOR THE BENEFIT OF GREATER CINCINNATI BEHAVIORAL HEALTH SERVICES.

THE PURPOSE OF THE HISTORIC VILLAGE IS TO DEPICT THE WAY OF LIFE OF RURAL INHABITANTS OF NORTHWEST OHIO, BETWEEN 1803 & 1949, THROUGH DEMONSTRATIONS, ACTIVITIES AND EXHIBITS. GUESTS ARE PRIMARILY FROM THE SURROUNDING TRI-STATE AREA.
