LPCCN IMPROVES ACCESS TO APPROPRIATE RURAL HEALTH SERVICES IN GENESEE ORLEANS AND WYOMING COUNTIES AND COORDINATES RESOURCES AT THE LOCAL LEVEL THROUGH AN INTEGRATED COMMUNITY SERVICES NETWORK.
