THE NURSING HOME PROVIDED LONG TERM SKILLED NURSING CARE TO THE COMMUNITY THROUGH MAY 7, 2014. THE NYS DEPARTMENT OF HEALTH HAS APPROVED THE CLOSING OF THE NURSING HOME FACILITY. PRIOR TO MAY 7, 2014, ALL NURSING HOME PATIENTS WERE TRANSFERRED TO OTHER FACILITIES. THE ENTITY DISSOLVED ON APRIL 29, 2021.
